Health Requirements
Student Health Services requires that all students submit documentation of their immunizations in accordance with Connecticut law. Online health forms, found in the student health portal are also required.
Instructions for completing health requirements
All students who will be on campus for any amount of time must complete the following:
All students must provide proof of compliance with State of Connecticut immunization requirements:
- Two doses of measles, mumps, and rubella (MMR) vaccine and two doses of varicella (chickenpox) vaccine are required for all students.
- Meningococcal ACYW vaccine given within the past 5 years is required for living in campus housing
- If you do not have your vaccine record you may submit a blood test proving immunity (called a titer) for MMR and varicella. There is no blood test to replace the meningococcal ACYW vaccine.
All students must access the student health portal to upload their complete immunization record and fill out the online forms required by Student Health Services.

The School of Nursing, Physician’s Assistant Institute, and Fones School of Dental Hygiene have additional immunization, blood tests, and physical exam requirements. Please contact your program directly if you have any questions.
Student athletes
Student Athlete Pre-Participation Physical Exam Form (PDF) - For students in any program that are joining a NCAA intercollegiate athletic team. This form is required prior to participation in any varsity sport. This does not include intramural, recreation or club sports.

Students will be responsible for obtaining these vaccines from an outside provider if necessary. Due to the high cost of these vaccines, please discuss these requirements with your primary care provider or your local health department.
Hepatitis B
A law enacted by the State of Connecticut mandates that all college students must be informed of hepatitis B infection and its devastating consequences. All clinical health science students are required to be vaccinated for hepatitis B or provide serologic evidence of immunity. Non-clinical students are strongly encouraged to be vaccinated for hpatitis B, which is administered as a series of three (3) injections over a six-month period.
